Amends the Public Health Code to require the Department of Health to maintain a list of reportable diseases, infections, and disabilities that is reviewed and revised at least annually, removing the requirement that the department promulgate rules for this function.

Clarifies definitions of communicable diseases, serious communicable diseases, HIV infection, venereal disease, and other health-related terms used in disease reporting and control provisions.

Establishes procedures for local health departments to furnish care to individuals with serious communicable diseases or infections (excluding HIV/AIDS) and requires reporting to county social services departments.

Maintains strict confidentiality protections for HIV and AIDS information, limiting disclosure to authorized purposes such as protecting individual health, preventing transmission, and patient diagnosis and care, with civil and criminal penalties for unauthorized disclosure.

Allows police officers, firefighters, correctional officers, and emergency responders to request testing of arrestees, inmates, and emergency patients for HIV, hepatitis B, and hepatitis C infections following potential bloodborne pathogen exposures, with confidentiality protections and notification requirements.
