Requires individuals to report in person to the Secretary of State within 7 days after being involved in a motor vehicle crash resulting in property damage of $1,000 or more, personal injury to any individual, or being stopped by a peace officer for a violation of this act.

Mandates individuals provide proof of vehicle insurance coverage at the time of the crash or stop, as required under Chapter 31 of the Insurance Code of 1956, with the Secretary of State verifying coverage with the stated insurer.

Directs the Secretary of State to report violations of insurance coverage requirements to the responding police or sheriff department, or to the appropriate jurisdiction if no officer responded to the incident.

Establishes criminal penalties of up to 93 days imprisonment and/or fines up to $500 for failure to report as required or for knowingly providing false or misleading information in reports.

Defines "crash" as a collision with any object including another vehicle, and "police department" to include city, village, township, college, community college police departments, and the state police department.
