Amends marriage license application requirements to include a space for parties to elect whether the marriage is a covenant marriage under the covenant marriage act.

Requires applicants for marriage licenses to provide their social security numbers on applications, but prohibits the state registrar from displaying social security numbers on the actual marriage license.

Makes marriage license applications nonpublic records exempt from disclosure under the Freedom of Information Act, available only to the named parties upon request.

Establishes criminal penalties for unauthorized disclosure of social security numbers collected for marriage applications: first violation is a misdemeanor punishable by up to 90 days imprisonment or $500 fine; second or subsequent violations are felonies punishable by up to 4 years imprisonment or $2,000 fine.

Exempts applicants from providing social security numbers if they are legally exempt from obtaining one or if they have religious convictions exempting them from disclosure.

Requires individuals officiating marriages to deliver any completed covenant marriage declaration of intent to the county clerk along with the completed marriage license.

Ties bill effectiveness to enactment of Senate Bill No. ____ or House Bill No. 5951.
