Establishes criminal penalties for violations of specific Public Health Code sections related to nursing homes, including imprisonment up to 1 year and/or fines of $1,000 to $10,000.

Creates civil liability for violations of section 21765a(1) or (2), requiring payment of treble damages (three times actual damages) or $1,000 whichever is greater, plus attorney fees and costs.

Imposes administrative penalties of $100 per affected patient or actual reimbursement for costs/injuries (whichever is greater), plus an additional penalty of up to $1,500 or $15 per patient bed (whichever is less) for section 20201 violations.

Makes it a misdemeanor for nursing home licensees, administrators, or employees to knowingly and willfully provide false statements to the department during surveys, evaluations, consultations, or complaint investigations, punishable by up to 1 year imprisonment and/or $1,000 to $10,000 fine.

Requires the Department of Community Health to establish rules for a quality of care allowance formula based on prior committee recommendations regarding nursing home reimbursement.
