Prohibits transfer of nursing home and home for the aged licenses to new owners; requires new applicants to apply for licensure and meet all compliance requirements before ownership change occurs.

Increases civil penalty for violations from $1,000 to $10,000 per violation and establishes felony penalties (up to 4 years imprisonment or $30,000 fine) for false statements in applications.

Defines "change of ownership" to include sales, partnership changes, corporate mergers, limited liability company membership changes, trust transfers, and changes of 30% or more in corporate stock ownership.
