School districts and public school academies offering interscholastic or competitive athletic programs must develop guidelines and create a concussion and head injury information fact sheet for coaches, athletes, and parents or guardians.

Before participation in any interscholastic or competitive athletic activity, students and their parents or legal guardians must receive the fact sheet and sign and return it to the school.

If a student suffers a concussion or head injury during an athletic activity, the student must be immediately removed from that activity.

A student cannot return to athletic activity until evaluated by a licensed health care provider trained in concussion and head injury management and cleared in writing to return, with volunteers permitted to serve as evaluators.

"Athletic activity" is defined as games, meets, competitions, practices, or tryouts sponsored by the school district or public school academy.
