Testing must occur within 24 hours of notification, with student-athlete or parent responsible for paying counseling and testing costs beyond the surcharge-funded portion.

Establishes progressive penalties: first offense suspends participation for 180 or 90 school days with required substance abuse counseling and weekly testing; second offense suspends for 360 or 180 school days with 16 weeks of counseling; third offense results in permanent ineligibility.

Requires student-athletes and parents to sign consent forms before participation if the school's athletic association has adopted the required testing policy.
