Requires school district and public school academy boards to include counseling for all athletes about the risks and consequences of using performance-enhancing substances in their athletic eligibility policies.

Mandates that pupils undergo screening for performance-enhancing substance use as part of the physical examination required before participation in interscholastic athletics.

Requires schools to submit an annual report to the Department of Community Health by July 1 detailing the number and percentage of athletes who tested positive for performance-enhancing substances during the preceding school year.

Directs the Department of Community Health to develop and make available a model reporting form for schools to use in submitting their screening results.

Defines "performance-enhancing substance" as any substance included on the list of performance-enhancing substances under section 1318 of the school code.
