Establish and maintain a public database of child care centers, family child care homes, and group child care homes accessible on the department's website at no charge

Database must include provider name, address, telephone number, hours of operation, license/registration numbers with effective and expiration dates

Include number and nature of any adverse actions taken against providers by the department

Include number and nature of special investigations classified as high risk by the department, remaining in database for duration of licensure or registration

Include notation when a special investigation is completed with no substantiated rule violations

Department must publicize database availability and access instructions through press releases and other media
