Establishes a statewide assessment program in the Department of Education to evaluate student progress in reading, mathematics, language arts, and other general subject areas (subject to restrictions on grade 6).

Requires assessments to cover all students annually in at least 2 elementary and middle school grade levels, with additional grades required if mandated by the No Child Left Behind Act of 2001.

Mandates that assessment contractors meet quality management standards including at least Level 2 of the Capability Maturity Model (Level 3 for assessments after 2005-2006 school year) and that contracts include specific deadlines with penalties for noncompliance.

Requires individual student reports identifying whether students met or failed to meet expectations for each standard, allowing parents and teachers to identify and remedy problems before grade advancement.

Prohibits grade 6 assessments from including a social studies component.
