Petition size set at 8-1/2 inches by 14 inches, arranged horizontally on the sheet.

Petition heading must be printed in 14-point boldfaced capital letters specifying whether it is an "INITIATIVE PETITION," "AMENDMENT TO THE CONSTITUTION," "INITIATION OF LEGISLATION," or "REFERENDUM OF LEGISLATION."

Full text of proposed amendment, law, or referendum must be printed on the petition in 8-point type; if too lengthy for the signature side, a brief objective statement of subject matter appears on the signature side with full text on the reverse or fold-over extension.

Alterations to existing law must show added language in capital letters and deleted language with strikethrough.

Petition must include warning statements in 12-point boldfaced type about penalties for false signatures, a notice that the proposal was written by the sponsor, a certificate of circulator, and 15 numbered signature lines (reducible to minimum 7 lines) with fields for printed name, address, zip code, and date.

Act takes effect November 15, 2010, contingent on enactment of Senate Bill 954.
