Former parents may file statements with the central adoption registry to consent to or deny release of identifying information, with the ability to file, update, or revoke statements at any time.

Adult former siblings may file statements with the registry consenting to release of their name and address to adult adoptees, and may file notice of a former parent's death with a death certificate.

Courts must inform parents at the time of termination of parental rights about the registry provisions and their right to deny release of identifying information, providing prescribed forms.

Child placing agencies, courts, and the department must provide nonidentifying information within 63 days and identifying information within 28 days, with release rules varying based on adoption date and parental consent status.

Employees or agents who intentionally release identifying information in violation of this section are guilty of a misdemeanor; fees for information are capped at $60 or actual cost, whichever is less.
