Department of Human Services, Department of Community Health, and Medical Services Administration shall collaborate to identify goals and benchmarks to reduce diabetes incidence, improve diabetes care, and control related complications.

Each of the three departments shall develop individual entity plans to address diabetes reduction, improved care, and complication control in Michigan.

By January 10 of each odd-numbered year, the three departments shall submit a collaborative report to the legislature containing financial impact assessments, program benefit evaluations, coordination levels, and a detailed diabetes action plan.

The report must document the number of people with diabetes covered by each department, funding sources and amounts directed toward diabetes programs, and financial impact compared to other chronic diseases.

The action plan shall identify proposed action steps to reduce diabetes impact, expected outcomes with benchmarks for the following biennium, and a detailed budget blueprint identifying costs and resources needed for implementation.
