Establishes state support for voluntary home visitation programs serving families with children ages 0-5 and pregnant women through periodic face-to-face visits by nurses, social workers, and trained professionals.

Limits state funding to evidence-based programs with rigorous peer-reviewed research demonstrating sustained positive outcomes, or promising programs with active evaluations and a transition plan to evidence-based status.

Requires supported programs to address maternal and child health, parenting practices, child development, school readiness, and family self-sufficiency through standardized program models with trained staff and community service linkages.

Directs the departments of community health, human services, and education to develop collaborative data sharing processes and include consistent home visitation language in contracts and grants.

Requires annual reports to legislature beginning December 1, 2013 detailing program outcomes, number of families served, funding allocation between evidence-based and promising programs, and cost per family metrics.
