Requires candidates for federal, state, county, city, township, village, and school district offices to file an affidavit of identity with 2 copies when filing nominating petitions, qualifying petitions, or filing fees.

Exempts candidates for federal office, Michigan Supreme Court justice, and Court of Appeals judge from the requirement to file documents in person with the receiving officer.

Mandates that candidates nominated at political party conventions or caucuses file an affidavit of identity with the Secretary of State within 1 business day of nomination.

Requires affidavits of identity to include candidate's name, address, voter registration information, citizenship status, residency duration, desired ballot name, and disclosure of any name changes (with exceptions for name changes over 10 years old, naturalization name changes, marriage, divorce, or common law names).

States that candidates filing for multiple concurrent offices must select one office within 3 days after the filing deadline or be disqualified from all offices and have their name removed from ballots.
