Adds Part 204 to the Public Health Code to establish requirements for health facilities and agencies regarding medical futility policies.

Defines "futility policy" as any written policy that allows health facility employees or medical professionals to withhold or discontinue treatment based on a judgment that further medical treatment would have no useful result.

Requires health facilities or agencies that adopt futility policies applicable to children ages birth to 18 years to disclose the policy to the child's parent or guardian when a formal process is needed to address treatment concerns.

Mandates that health facilities or agencies provide a copy of their futility policy to parents or guardians of patients or prospective patients upon request.
