Amends the Public Health Code to require nursing homes to execute written contracts with patients or their legal representatives at admission, contract expiration, or when payment source changes.

Requires nursing home contracts to be written in clear language, printed in at least 12-point type, and include specific provisions regarding term, services, charges, payment sources, deposits, patient rights, and hospice care availability.

Mandates nursing homes provide patients and their representatives with a copy of the contract at execution and make copies available to persons funding care and the Department of Community Health for publicly-funded patients.

Requires nursing homes to notify patients in writing about proposed sales or changes in ownership or control interest, and to include notice of noncompliance issues that could delay licensing approval for new owners.

Expands nursing home licensing posting requirements to include names and contact information for persons with ownership interests, facility administrators and managers, and copies of proposed ownership change reports filed with the department.
