Creates a "Peace of Mind Registry" to store and provide access to patient directives, including durable powers of attorney for health care and anatomical gift documents.

Requires the registry to be accessible to registrants, health care providers, and the Department of Community Health through secure user identification and password, with continuous free electronic access for health care providers.

Allows directives to be submitted by mail or secure electronic format (PDF), with the most recently signed directive superseding earlier versions.

Mandates the Department of Community Health, Secretary of State, and Department of Human Services to promote public awareness of directives and the registry through their websites.

Requires annual reporting to legislative health policy committees by January 31 on total current and new registrants, and grants civil immunity to contracted registry organizations except in cases of willful or gross negligence.
