Changes the grade level for initial immunization reporting from grade 6 to grade 7, effective beginning in the 2013-2014 school year.

Requires districts and intermediate districts to report immunization status to local health departments by November 1 and February 1 each fiscal year for all entering students in grades K-12 and students newly enrolling in grade 7.

Establishes a 90% immunization compliance threshold for November 1 reports and a 95% threshold for February 1 reports; districts failing to meet these thresholds are subject to funding penalties.

Authorizes the department to withhold 5% of state school aid funds from noncompliant districts until they comply; any withheld funds are forfeited if compliance is not achieved by fiscal year end.

Becomes effective July 1, 2012, contingent upon enactment of Senate Bills 1086 and 1088.
