Changes the grade level requirement for immunization documentation from grade 6 to grade 7, effective beginning the 2013-2014 school year.

Requires children enrolling in grade 7 for the first time to submit immunization records, physician statements, religious conviction statements, or documentation of compliance in progress.

Requires kindergarten enrollees to submit vision screening test results from a health department or eye examination documentation from a licensed physician or optometrist conducted after age 3.

Mandates schools report immunization and vision screening status to the Department of Community Health by November 1 annually, with updates due by February 1.

Takes effect July 1, 2012, contingent upon enactment of Senate Bills 1086 and 1087.
