Amends section 11123 of the Natural Resources and Environmental Protection Act to modify operating license requirements for hazardous waste treatment, storage, and disposal facilities.

Changes criminal conviction disclosure requirements to include convictions for violations of any federal, state, Canadian, or Canadian provincial statute that is an environmental statute or where the violation was committed in furtherance of obtaining the permit.

Establishes application fee of $25,000.00 plus facility-type specific fees ($9,000 for landfills/surface impoundments; $7,200 for incinerators/treatment facilities; $500 for storage facilities).

Requires applicants to publish notice in newspapers at least 30 days before submission and conduct public preapplication meetings, with summary of comments and responses included in application.

Requires comprehensive disclosure statement listing applicant, major equity holders, operator, key employees, environmental permits held, and activities resulting in environmental threats funded by public money.
