Establishes ambulance operation licensing requirements, with a $100 license fee and annual $100 renewal fee administered by the state health department.

Allows ambulance operations in counties or micropolitan areas with populations of 10,000 or less and population density less than 7 people per square mile to operate ambulances with less than 24-hour daily availability, subject to medical control authority authorization.

Permits basic ambulance operations in low-population areas to operate at increased licensure levels (limited advanced or advanced life support) when staffed with an EMT-specialist or paramedic, if equipment and medical protocols support the higher level.

Requires ambulances to be staffed minimally with: 1 EMT and 1 medical first responder (basic), 1 EMT-specialist and 1 EMT (limited advanced), or 1 paramedic and 1 EMT (advanced life support).

Repeals new section 20921a effective January 1, 2018, making the rural ambulance flexibility provisions temporary for approximately 3 years.
