Acknowledgment of parentage forms must be signed by both mother and father, with signatures notarized by a notary public or witnessed by one disinterested, legally competent adult employed by specified entities (hospitals, health clinics, courts, Department of Human Services, county jails, state prisons, and similar organizations).

Witness must sign and date the acknowledgment form and provide printed name, address, and place of employment.

State registrar must review completed acknowledgments for proper completion and notarization/witness requirements before filing in the parentage registry as a permanent record.

Completed and filed acknowledgment forms may serve as basis for preparation of a new birth certificate.
