Individuals applying for assistance under the Social Welfare Act must sign an affidavit confirming their school-aged child is enrolled and attending school with at least 80% attendance rate for the current school year (excluding illness or injury-related absences).

The affidavit must include the child's name, school name, and school district name, and the department may require consent to release school attendance records as a condition of eligibility.

The department shall verify with schools during application and renewal that children listed on affidavits are currently attending and maintaining at least 80% attendance for the current school year (excluding illness or injury-related absences).

Recipients remain eligible for assistance if their school-aged child meets the 80% attendance requirement and all other eligibility requirements are satisfied.

As a condition of eligibility, individuals with school-aged children attending school outside the home must attend at least one parent-teacher conference during the school year they receive assistance.
