Expands the definition of "personal injury" for firefighters, police officers, sheriffs, state police, conservation officers, and motor carrier enforcement officers to include respiratory and heart diseases that develop during active service and result from job duties.

Creates a presumption that certain cancers (respiratory tract, bladder, skin, brain, kidney, blood, thyroid, testicular, prostate, and lymphatic) in firefighters with 60+ months of service arise from employment, unless rebutted by evidence of substantial tobacco use within 10 years before diagnosis.

Establishes the "first responder presumed coverage fund" as a separate state treasury fund to pay cancer-related claims, with claims reviewed and approved or denied within 30 days of receipt.

Requires firefighters and other covered employees to apply for pension benefits first and receive a pension denial before claiming workers' compensation benefits, though pension eligibility does not prevent claiming medical expense benefits.

Mandates annual reporting to the legislature on fund claims, approvals, payments, and administration costs, with the director notifying the legislature if funds become insufficient within 60 days.
