Defines total annual costs to include premiums, employer reimbursements for copays and deductibles, and health savings account contributions, but excludes employee out-of-pocket expenses

Permits public employers to allocate the employee cost-share among employees as they determine appropriate

Effective December 30, 2013, applies to medical benefit plan coverage years beginning on or after January 1, 2012
