Physicians performing abortions must report the procedure to the state health department within 7 days using department-prescribed forms.

Abortion reports must contain only specified demographic and medical information (age, marital status, race, ethnicity, facility location, pregnancy history, abortion method, fetal weight, evidence of life, complications, payment method) and exclude patient identifying information.

The department must destroy individual abortion reports after retaining them for 5 years and publish aggregate annual statistics on gestation period, abortions on minors, and physical complications.

Unauthorized disclosure of confidential identifying information about individuals obtaining abortions is a felony punishable by up to 3 years imprisonment or $5,000 fine.

Any individual with knowledge of a woman seeking treatment for abortion-related physical complications may file a notice of compliance with the department, which must be retained for 5 years; knowingly filing a false notice is a misdemeanor punishable by up to 1 year imprisonment or $1,000 fine.
