Amends the state personal identification card statute to add a medical information registry option, allowing cardholders to register medical information through section 310a of the Michigan vehicle code.

Requires the Secretary of State to provide applicants with information about both the anatomical gift donor registry and the medical information registry, with opportunities to enroll in either or both programs.

Establishes that the Secretary of State shall maintain records of individuals enrolled in the medical information registry and maintain the registry in compliance with state law, including electronic access for authorized users.

Clarifies that enrollment in either the donor registry or medical information registry remains binding after card expiration and that individuals are not considered to have revoked their enrollment solely because their identification card expired.

Adds new subsection (9) requiring the Secretary of State to maintain records of medical information registry enrollees and subsection (21) requiring inquiries about participation in both the donor registry and medical information registry during card applications and renewals.
