Allows municipal health facilities corporations and subsidiary corporations in counties with populations between 26,000 and 30,000 (based on 2010 census) to restructure as nonprofit corporations under the Nonprofit Corporation Act.

Changes the restructuring deadline from July 1, 2012 to June 30, 2017.

Requires a restructuring plan that includes the terms and conditions of the restructuring and proposed articles of incorporation and bylaws compliant with the Nonprofit Corporation Act.

Requires prior majority approval from the county board of commissioners, city council, or village council before restructuring can occur.

Preserves all existing liabilities, obligations, property rights, and legal proceedings of the original corporation, treating the restructured entity as a continuation of the original corporation.
