Removes the 2014-2015 school year effective date restriction, allowing children enrolling in grade 7 for the first time (not just those beginning in 2014-2015) to submit immunization documentation.

Replaces the blanket religious exemption option with a requirement that parents submit either a medical contraindication form or nonmedical immunization waiver form from the Department of Health and Human Services for any immunizations not documented.

Changes references from "Department of Community Health" to "Department of Health and Human Services" throughout the statute.

Requires schools to report immunization status of first-time enrolling students in grades K-12 (and grade 7 first-time enrollees) to the health department by November 1, with updates due by February 1, using forms approved by the department.

Takes effect 90 days after enactment, contingent upon House Bill 5361 being enacted into law.
