Establishes "Physician Orders for Scope of Treatment" (POST) forms as a new medical document allowing patients with advanced illness to specify types and levels of treatment they wish to receive outside of hospital settings.

Requires the Department of Public Health to develop a standardized POST form with specific format, color, and content requirements including patient information, signatures, medical orders, and statements about voluntary consent and form validity.

Allows patients, patient advocates, or guardians to consent to POST form orders; permits revocation at any time by patients or patient representatives through written notice to health professionals.

Protects health professionals and facilities from criminal, civil, or professional liability when providing treatment consistent with POST forms or without actual notice of revocation, except when treatment contradicts accepted health care standards.

Prohibits health facilities from requiring POST form execution as a condition of admission and prevents life insurers from denying coverage, charging higher premiums, or invoking suicide exemptions based on POST form execution.
