Amends the Michigan personal identification card law to allow applicants to request that a mailing address appear on their identification card instead of their residence address.

Official state personal identification cards must contain identification number, full legal name, date of birth, sex, address, height, weight, eye color, digital photo, signature, and expiration date, with optional blood type, immunization data, medication data, and deaf status indicators.

Establishes a $10.00 fee for original or renewal identification cards, with waivers for seniors (65+), individuals with suspended/revoked licenses due to disability, those with statutory blindness, and those requesting only donor registry changes.

Identification cards expire on the cardholder's birthday in the fourth year after issuance; renewal by mail allowed for one additional 4-year period, except sex offenders must renew in person.

Requires Secretary of State to establish an anatomical gift donor registry by January 1, 2017, allowing cardholders to voluntarily participate, with enrollment constituting a binding legal agreement that survives card expiration and overrides family objections.
