Requires candidates filing for federal, state, county, city, township, village, metropolitan district, or school district offices to submit 2 copies of an affidavit of identity with their nominating petition or filing fee.

Affidavit of identity must include candidate's name, address, ward and precinct, citizenship status, state and county residency duration, and manner name should appear on ballot.

Requires candidates to disclose whether they are using a name different from their birth name, including their full former name, unless the name change occurred at least 10 years prior, through marriage, divorce, naturalization, or common law usage.

Adds a checkbox option allowing candidates to prevent publication or disclosure of their residence address.

Affidavit must include statement that all campaign finance reports, filing fees, and fines under the Michigan Campaign Finance Act have been filed or paid, and acknowledge that false statements constitute perjury punishable by up to $1,000 fine or 5 years imprisonment.
