Amends the State School Aid Act to require school districts and intermediate districts to report pupil immunization status to local health departments by November 1 and February 1 of each fiscal year for students in grades K-12 entering for the first time or entering grade 7 for the first time.

Requires the Department of Community Health to report back to the Department of Education by December 31 and March 31 each year specifying the percentage of entering pupils without completed, waived, or provisional immunization records, with compliance thresholds of 90% by the first report and 95% by the second report.

Districts and intermediate districts must post immunization reporting information on their public websites or in another public manner prescribed by the Department of Community Health.

Withholds 5% of state school aid funds from districts or intermediate districts that fail to comply with reporting requirements, with the full withheld amount forfeited if compliance is not achieved by the end of the fiscal year.

Takes effect 90 days after enactment and is contingent upon passage of Senate Bill No. 259 of the 98th Legislature.
