Amends the Public Health Code to revise Section 5111 regarding the department's authority to maintain and review a list of reportable diseases, infections, and disabilities at least annually.

Authorizes the department to promulgate rules establishing reporting requirements and surveillance methods for communicable, serious communicable, chronic, and noncommunicable diseases, with reports on serious communicable diseases or infections required within 24 hours of diagnosis.

Requires the department to establish procedures for investigating cases and epidemics, controlling diseases through immunization and environmental controls, preventing and treating disabilities, controlling rabies, and reporting lead poisoning cases.

Mandates the department to promulgate rules ensuring confidentiality of reports, records, and data related to testing, care, treatment, reporting, and research associated with communicable and serious communicable diseases or infections.

Takes effect July 4, 2016, and is contingent upon enactment of Senate Bills 374, 376, 377, 378, 379, 380, and 381.
