Amends the Revised School Code to modify immunization requirements for students enrolling in public or nonpublic schools for the first time or entering grade 7 for the first time.

Removes the 2014-2015 school year effective date restriction, making immunization submission requirements apply to all new enrollees regardless of school year.

Requires students to submit one of three documents: physician statement of immunization, parent/guardian statement of religious objection (referencing Public Health Code section 9215), or physician statement that child is in process of complying with immunization requirements.

Requires kindergarten enrollees to submit vision screening documentation from health departments, licensed physicians, or optometrists, unless parent/guardian provides religious conviction objection statement.

Schools must report immunization status and vision screening data to the Department of Health and Human Services by November 1 annually, with February 1 updates.

Takes effect 90 days after enactment only if Senate Bill 1040 is also enacted into law.
