Establishes Physician Orders for Scope of Treatment (POST) forms as standardized medical documents for patients with advanced illness to communicate treatment preferences, distinct from advance health care directives.

Requires the Department of Health to develop a standardized POST form on specific colored paper including patient information, medical orders, signatures, and annual review requirements; also requires an accompanying information form explaining the POST process.

Allows patients or their authorized representatives (patient advocates or guardians) to consent to POST forms after consultation with an attending health professional; patients may revoke POST forms at any time through any communicable method.

Directs emergency medical services personnel to follow POST form medical orders unless the orders conflict with accepted health care standards, emergency protocols, or a newer do-not-resuscitate order exists, and prohibits discrimination by life and health insurers based on POST form execution.

Prohibits health facilities and agencies from requiring POST form execution as a condition of admission or services; protects health professionals from liability for complying with or not complying with POST orders based on lack of notice or good faith standards.
