Amends Section 1177 of the Revised School Code to clarify immunization requirements for children enrolling in public or nonpublic schools for the first time or enrolling in grade 7 for the first time.

Requires submission of one of three documents: physician certification of immunization/testing, parental statement of religious/other objection to immunization per Public Health Code Section 9215, or physician statement that child is in process of complying with immunization requirements.

Requires kindergarten enrollees to submit vision screening documentation from health department or licensed physician/optometrist, with exemption available via parental statement of religious convictions.

Mandates schools report immunization status to the Department of Health and Human Services by November 1 annually for new students in grades K-12, with updated reports due by February 1.

Takes effect 90 days after enactment and is contingent upon passage of a related bill (referenced as Senate Bill No. ____ or House Bill No. 4425).
