Amends the Dog Law of 1919 to allow dog license notations indicating the dog is a service animal, emotional support animal, or therapy animal upon applicant request.

Requires service animal applicants to submit a signed affidavit attesting the dog has been individually trained to perform tasks for a person with a disability as defined in the Americans with Disabilities Act.

Requires all applicants for service, emotional support, or therapy animal notations to provide a basic obedience program certificate of completion from a certified instructor or equivalent as determined by the Department of Agriculture and Rural Development.

Makes it a misdemeanor punishable by up to 93 days imprisonment, a fine of up to $500, and/or up to 30 days community service to knowingly submit false or fraudulent affidavits or obedience certificates.

Takes effect 90 days after enactment and is conditioned on passage of House Bill 5281 or a companion Senate bill.
