Amends MCL 333.16226 to establish a comprehensive table of disciplinary sanctions that disciplinary subcommittees must impose for violations of section 16221 of the Public Health Code, organized by violation type.

Creates mandatory sanction schedules ranging from reprimands and fines to suspension, revocation, and permanent revocation depending on the specific violation committed by licensed or registered health professionals.

Authorizes disciplinary subcommittees to impose fines up to $250,000 for violations of section 16221(a) or (b), with a minimum fine of $25,000 if the violation results in patient death.

Requires permanent revocation for violations of section 16221(b)(xiii) when the violation occurs while the licensee was acting within their licensed health profession.

Limits permanent revocation sanctions (except in specific cases) to situations where the subcommittee finds a pattern of intentional fraud or deceit resulting in financial gain to the licensee and harm to patient health, and takes effect 90 days after enactment only if HB 5787 and HB 5789 are also enacted.
