Establishes annual fees for community water supply providers based on number of residents served, ranging from $250 for providers serving 25-100 residents to $83,800 for those serving over 500,000 residents.

Requires fees to be adjusted annually on October 1 using the Detroit Consumer Price Index, with additional adjustments possible based on changes in federal funding or actual costs.

Creates the "Rebuild Michigan Fund" to receive revenues from community water supply fees, with funds to be retained in state treasury and expended only to implement the Safe Drinking Water Act and its administrative rules.

Requires community supply providers serving 1,000 or more residents to submit annual inventory reports by September 1, 2019 and each subsequent January 1 detailing meter sizes for residential and nonresidential accounts.

Establishes a water assistance grant program administered by the Department of Treasury to fund low-income household water assistance programs for households earning not more than 150% of federal poverty guidelines.
