Health insurers in Michigan must offer coverage for medically necessary prosthetic devices, components, and their repair, fitting, and replacement on all health policies delivered, issued, or renewed in the state.

Coverage cannot be subject to separate annual or lifetime dollar maximums beyond those that apply to all items and services under the policy, and cannot have deductibles or copayments greater than those for physical illness generally.

Amounts paid for prosthetic devices cannot be applied toward dollar maximums for other durable medical equipment, except aggregate maximums covering all policy benefits.

Coverage excludes repairs and replacements due to insured neglect, misuse, abuse, or prosthetic devices designed primarily for athletic purposes.

The requirement applies to policies delivered, issued, or renewed on or after January 1, 2019, or when any policy term is changed or premium adjusted after that date.
