Amends Michigan's Public Health Code to require reporting of HIV test results to local health departments or the state department of health within a timeframe determined by the department.

Requires reported information to include test subject demographics (name, address, telephone, date of birth, race, sex), healthcare provider details, specimen collection date, test type, test result, transmission method, and other epidemiological data deemed necessary by the department.

Allows individuals tested for HIV in a physician's private practice or health maintenance organization to request anonymity by requesting their name, address, and telephone number be excluded from the report.

Physicians must comply with anonymity requests and submit specimens to laboratories without the test subject's identifying information, except as otherwise provided in section 5114a.

Takes effect 90 days after enactment into law.
