Allows state and local registrars to conduct free searches for live birth records and issue certified copies or official statements without charging fees when individuals execute an affidavit under penalty of perjury certifying need for obtaining or renewing state ID, operator's license, or chauffeur's license.

Requires affiant to certify they have not possessed a valid state ID or driver's license within the 4 years preceding the request, or that the Secretary of State will not renew their ID/license without presenting the birth record.

Requires affiant to certify the cost of obtaining the record is prohibitive.

Appropriates $2,000,000 to the Department of Health and Human Services for the 2017-2018 fiscal year to issue certified copies of live birth records under this provision and implement the new section.

Makes various technical amendments to MCL 333.2891 regarding vital records fees and procedures, changing "person" to "individual" throughout and clarifying registrar authority to issue official statements when records cannot be located.

Takes effect 90 days after enactment, contingent on enactment of HB 6046 and HB 6048.
