Establishes felony penalty for signing any petition (nominating, qualifying, political party formation, or recall) with multiple names, with signatures by violators declared invalid and uncounted.

Creates misdemeanor penalties of up to $500 fine or 93 days imprisonment for signing petitions with false names, making false statements on petition certificates, or falsely signing as a circulator.

Adds new section 482e establishing identical petition fraud prohibitions and penalties for petitions under section 482, with felony charges for multiple-name violations.

Allows school board candidates to pay $100 nonrefundable filing fee as alternative to collecting nominating petition signatures, with fee payment having same effect as petition filing.

Requires candidates filing affidavit of identity to disclose any name changes unless changed at least 10 years prior, through naturalization, marriage, or divorce to a previously known legal name, and mandates reporting of campaign finance compliance status and campaign finance violations.
