Candidates filing for federal, state, county, city, township, village, metropolitan district, or school district office must file 2 copies of an affidavit of identity, with exemptions for presidential and vice presidential candidates.

Affidavit of identity must contain candidate's name and residential address, U.S. citizenship statement, office title sought, statement of constitutional and statutory qualification, and desired ballot name format.

Candidates using a name other than their birth name must disclose their full former name, with exceptions for names changed 10+ years prior, through naturalization, marriage, divorce, or common law names.

Affidavit must include statements confirming all campaign finance reports, late fees, and fines have been filed or paid, and acknowledge that false statements constitute perjury punishable by up to $1,000 fine or 5 years imprisonment.

Officers must not certify candidates to the board of election commissioners who fail to file the affidavit or who file an affidavit containing false information; perjury violations are separate from campaign finance act violations.
