Removes the 2014-2015 school year implementation date and applies immunization submission requirements to all children enrolling in public or nonpublic schools for the first time or enrolling in grade 7 for the first time.

Requires submission of one of three documents: physician statement of immunization, parent/guardian statement of religious or other objection to immunization as described in MCL 333.9215, or physician statement that child is in process of complying with immunization requirements.

Requires kindergarten enrollees to submit vision screening documentation from a health department or examination by a licensed physician or optometrist, with religious conviction exemption available.

Mandates schools report immunization status to the Department of Health and Human Services by November 1 annually and provide updates by February 1 for pupils in grades K-12 who enrolled for the first time.

Takes effect 90 days after enactment and is contingent on Senate Bill 300 of the 99th Legislature being enacted into law.
