Expands the definition of "personal injury" for fully paid firefighters, police officers, sheriffs, state police, conservation officers, and motor carrier enforcement officers to include respiratory and heart diseases that develop during active service and result from job duties.

Creates a presumption that certain cancers (respiratory tract, bladder, skin, brain, kidney, blood, thyroid, testicular, prostate, breast, and lymphatic) arising in firefighters and fire authority members with 60+ months of service are work-related occupational injuries.

Establishes the "first responder presumed coverage fund" as a separate state treasury fund to pay workers' compensation benefits for cancer claims, with the fund receiving appropriations from the legislature.

Requires claimants to first apply for and exhaust eligible pension benefits before receiving benefits from the presumed coverage fund, though employees eligible for pensions may still receive additional medical expense coverage.

Takes effect 90 days after enactment and requires the workers' compensation agency to submit annual reports to legislative appropriations committees on claims received, approved, paid amounts, and anticipated funding needs.
