Water and sewerage service providers must file annual reports with the Michigan Department of Health and Human Services by April 1 each year for the previous calendar year.

Annual reports must include rate categories, rates charged by customer type, average monthly bills for households of 2, 4, and 6 individuals, and the process used to determine rates.

Providers must report total shutoffs and shutoffs broken down by census tract, zip code, residential customers, commercial customers, senior citizens, customers with emergency medical conditions, customers with disabilities, and customers with young children or who are pregnant.

Reports must detail the provider's shutoff policies and procedures, and identify any assistance programs available for low-income customers.

Providers must make annual reports available to customers on their website and post printed copies in their offices.
