Michigan Domestic and Sexual Violence Prevention and Treatment Board must create a standard police sexual misconduct incident report form by December 31, 2018, containing victim information, witness details, alleged officer identification, and description of the alleged misconduct.

Department of State must make the form available at all branch offices and self-service stations by March 31, 2019; Department of Health and Human Services must make it available at all offices by the same date.

Health care facilities providing sexual assault evidence kits must provide the form to victims and forward completed forms to the Department of Health and Human Services beginning March 31, 2019.

Department of State and Department of Health and Human Services must forward completed reports to the identified law enforcement agency or to the State Police if no agency is identified.

State Police must make reasonable efforts to identify the employing law enforcement agency and forward the report accordingly, or notify the victim if identification is impossible; bill takes effect 90 days after enactment and is contingent on Senate Bill 968 also being enacted.
